Metatarsal Fracture (5th Metatarsal / Jones & Stress Fracture)Foot Metatarsal Bone Fracture (Metatarsal Fracture (Jones / Stress Fracture))
Metatarsal Fracture (5th Metatarsal / Jones & Stress Fracture) — Overview
Metatarsal fractures disrupt the anterior weight-bearing tripod of the foot. Fractures of the proximal 5th metatarsal require strict anatomical differentiation between Zone 1 (tuberosity avulsion by peroneus brevis/lateral plantar aponeurosis), Zone 2 (Jones fracture at the metaphyseal-diaphyseal junction—a vascular watershed area with high non-union risk), and Zone 3 (diaphyseal stress fracture).
A metatarsal fracture (foot bone / 5th metatarsal Jones fracture) is a break in one of the পাঁচ long arch bones in the middle of your foot that carry your weight every time you step and push off.
Primary Causes & Biomechanics
Key Points You Should Know
- •Inversion ankle roll avulsing the 5th metatarsal styloid (Zone 1), adduction load on a plantarflexed forefoot (Zone 2 Jones), or repetitive marching/running overload ('March fracture' of the 2nd/3rd metatarsal neck)
- •Precarious retrograde nutrient artery blood supply in Zone 2 of the 5th metatarsal
- •Loss of intrinsic foot arch support and calf atrophy following walking-boot immobilization
- •Why the Outer Foot Bone Needs Extra Care: A break slightly further down the outer edge of the foot (Jones fracture) has delicate blood flow and needs strict protection from early tiptoe pushing.
- •Wearing Your Walking Boot/Stiff Shoe: A rigid sole prevents the front of your foot from bending upward and pulling the healing bone apart.
- •Rebuilding the Foot Arch: Once healed, arch-doming and calf exercises restore shock absorption.
Assessment Methods
1. Goniometric ROM & Postural / Kinematic Analysis
Ankle dorsiflexion/plantarflexion, subtalar inversion/eversion, and 1st–5th metatarsophalangeal (MTP) extension required for push-off.
2. Special Clinical Provocation & Neurological/Functional Tests
Palpation of the 5th metatarsal base (Zones 1–3) vs. distal shaft, tuning-fork/hop test (for occult stress fractures), and midfoot Lisfranc stability check.
3. Standardized Clinical Outcome Scales & Imaging Criteria
AOFAS Midfoot/Forefoot Scale, FAAM, and 3-view weight-bearing foot radiographs (AP, 45° oblique, lateral).
1. Range of Motion & Posture Check
Your physiotherapist gently evaluates how comfortably and smoothly your metatarsal bones (1st–5th metatarsal shaft/neck and 5th metatarsal base Zone 1 avulsion, Zone 2 Jones fracture, and Zone 3 stress fracture) moves and checks your overall posture.
2. Muscle Strength & Functional Movement Check
Your muscle balance, walking or reaching comfort, and specific gentle movement tests are evaluated without triggering sharp pain.
3. Typical Symptoms & Daily Impact Review
Localized swelling, bruising, and pinpoint pain along the outer edge of the foot (5th metatarsal) or over the top of the forefoot (stress fracture) that worsens with forefoot push-off.
Treatment Options & Phase-Based Protocol
✨ Enriched GIF Exercise & Precaution GuidePhase 1: Acute Symptom Modulation, Tissue Protection & Early Activation
- •Phase 1 Clinical Focus & Tissue Protection: Immobilization & Offloading Phase (Weeks 0–6): CAM walking boot or hard-soled shoe (strict NWB for 6–8 weeks in Zone 2 Jones fractures!), active ankle pumps (when out of boot if stable), and proximal leg strengthening (quad sets, SLR, side-lying hip abduction).
- •Pain, Edema & Neuromuscular Modulation: Targeted physical modalities, relative load management, and early protected mobility of the metatarsal bones (1st–5th metatarsal shaft/neck and 5th metatarsal base Zone 1 avulsion, Zone 2 Jones fracture, and Zone 3 stress fracture) without provoking mechanical or inflammatory irritation.
- •Proximal & Distal Kinetic Chain Maintenance: Maintain active mobility and circulation in adjacent joints while protecting the primary healing structures in Metatarsal Fracture (Jones / Stress Fracture).
Phase 2: Progressive Mobility, Motor Control & Targeted Strengthening
- •Phase 2 Progressive Mobilization & Motor Control: Post-Boot Mobility & Intrinsic Arch Phase (Weeks 6–8): Towel-assisted calf stretching, Short-Foot intrinsic arch activation & towel curls, and stiff-soled shoe transition with longitudinal/metatarsal arch support.
- •Biomechanical Alignment & Co-Contraction: Restore balanced agonist-antagonist force couples, joint centration, and multi-planar neuromuscular control across the metatarsal bones (1st–5th metatarsal shaft/neck and 5th metatarsal base Zone 1 avulsion, Zone 2 Jones fracture, and Zone 3 stress fracture).
- •Myofascial Extensibility & Endurance: Address secondary compensatory tightness and build local muscular endurance within safe, symptom-free biomechanical parameters.
Phase 3: Functional Integration, Proprioception & Long-Term Prevention
- •Phase 3 Functional Integration & Long-Term Prevention: Progressive Forefoot Loading & Return to Gait (Week 8–12+): Graduated bilateral-to-unilateral heel raises (calf raises), single-leg proprioceptive balance, and full push-off restoration.
- •Proprioceptive Balance & Dynamic Loading: Progress to task-specific closed-chain, balance, and functional movement patterns tailored to daily life, occupational, or athletic demands.
- •Self-Management & Relapse Prophylaxis: Establish lifelong ergonomic habits, home exercise maintenance, and early warning-sign awareness to prevent recurrence.
Step 1: Calming Pain, Protecting the Area & Gentle Starter Motion
- •Why the Outer Foot Bone Needs Extra Care: A break slightly further down the outer edge of the foot (Jones fracture) has delicate blood flow and needs strict protection from early tiptoe pushing.
- •Calm Protection & Gentle Start: During the initial stage of Foot Metatarsal Bone Fracture, avoid painful overloading and perform the gentle, pain-free starter movements shown below to support circulation and comfort.
- •Respecting Your Body's Signals: Every movement should stay within a comfortable, non-painful zone—never force through sharp or radiating pain.
Step 2: Restoring Flexibility & Building Supporting Muscle Strength
- •Wearing Your Walking Boot/Stiff Shoe: A rigid sole prevents the front of your foot from bending upward and pulling the healing bone apart.
- •Step-by-Step Flexibility & Support: As acute sensitivity settles, gradually build mobility and strengthen the supporting muscles around the joint at a steady, controlled pace.
- •Posture & Breathing Coordination: Keep your posture upright and breathe smoothly (never hold your breath!) during every repetition.
Step 3: Safe Daily Habits, Stamina & Preventing Recurrence
- •Rebuilding the Foot Arch: Once healed, arch-doming and calf exercises restore shock absorption.
- •Confident Return to Daily Life: Integrate your stronger muscles and improved balance into walking, stairs, work, and recreational activities.
- •Preventing Future Flare-Ups: Continue your core protective habits and key maintenance exercises even after symptoms resolve.

🔍 Enlarge1. Rhythmic Active Ankle Dorsiflexion & Plantarflexion Pumping (Venous/Edema Pump)
1. Ankle Toe-Pulls and Pointing (Circulation & Swelling Pump)
Rhythmic alternating contraction of the tibialis anterior and gastrocnemius-soleus complex. Activates the deep venous calf pump to prevent deep vein thrombosis (DVT), clears distal edema, and maintains talocrural mobility. Specifically indicated in Metatarsal Fracture (Jones / Stress Fracture) rehabilitation to optimize metatarsal bones (1st–5th metatarsal shaft/neck and 5th metatarsal base Zone 1 avulsion, Zone 2 Jones fracture, and Zone 3 stress fracture) biomechanics and neuromuscular control.
While lying down or sitting with your legs supported, pull your toes and feet firmly upward toward your shins, hold for 3 seconds, then point them downward like pressing a gas pedal. Perform smoothly within a comfortable, pain-free range to support your Foot Metatarsal Bone Fracture recovery.
🔍 Enlarge2. Intrinsic Foot Core ('Short Foot') & Medial Longitudinal Arch Activation
2. Seated Arch Doming (Short Foot) & Towel Scrunch
Drawing the first metatarsal head toward the calcaneus to elevate the medial longitudinal arch without curling the toes (or performing controlled towel curls). Strengthens the intrinsic plantar muscles supporting the subtalar and midtarsal joints. Specifically indicated in Metatarsal Fracture (Jones / Stress Fracture) rehabilitation to optimize metatarsal bones (1st–5th metatarsal shaft/neck and 5th metatarsal base Zone 1 avulsion, Zone 2 Jones fracture, and Zone 3 stress fracture) biomechanics and neuromuscular control.
While seated with your foot flat on the floor (or on a small towel), gently draw the ball of your foot toward your heel to lift your inner arch without curling your toes, hold for 5 seconds, and relax. Perform smoothly within a comfortable, pain-free range to support your Foot Metatarsal Bone Fracture recovery.
🔍 Enlarge3. Long-Sitting Towel-Assisted Gastrocnemius-Soleus & Plantar Fascia Stretch
3. Seated Towel Calf & Foot Arch Stretch
Long-sitting with a towel looped around the metatarsal heads, applying a slow, sustained passive pull into ankle dorsiflexion.
